Description

Job Description:
The Physical Therapist (PT) is responsible for evaluating and treating patients through medically prescribed therapy programs to restore mobility, improve function, and minimize disability following illness or injury.

Responsibilities include developing treatment plans, documenting patient progress, completing evaluations, coordinating care with the clinical team, and supervising Physical Therapy Assistants when needed, while supporting the goals and mission of the organization.

Territory Details: San Francisco, Brisbane, Daly City, Colma, South SF, Pacifica, Millbrae, Hillsborough, Burlingame, San Mateo and neighboring cities as needed.

Requirement: One year of Home Health experience
