Description
Hospice RN
Location: Missoula, MT 59808
Assignment Length: 13 weeks
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (On-call and weekend rotation required)
Qualifications
- Current RN license in the State of Montana (BSN preferred).
- Valid driver’s license, appropriate auto insurance, and reliable transportation.
- Minimum of 2 years recent experience in acute care, home care, or hospice.
- Clinical skills preferred: venipuncture, catheterization, wound care, parenteral therapy.
- HCHB (Homecare Homebase) experience strongly preferred.
Essential Duties & Responsibilities
- Provide skilled nursing care and coordinate services for patients and families in residential settings, following physician authorization and team-based care plans.
- Observe, assess, and evaluate patient conditions, including physical and psychosocial status, and plan interventions accordingly.
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care.
- Maintain continuity of care through effective communication, case conferences, and collaboration with interdisciplinary teams and community agencies.
- Deliver quality symptom management, health education, counseling, and appropriate referrals to promote optimal patient outcomes and quality of life.
- Document accurate, current, and complete patient records per agency standards, OASIS requirements, and reimbursement regulations.
- Participate in team scheduling, on-call rotation, and respond to patient needs via telephone or home visits as required.
- Engage in quality improvement activities and attend in-service training sessions.
- Perform other duties as assigned by supervisor.
