Bilingual Healthcare Case Manager Summary: Remote/telephonic role with local field visits (~75% travel for face-to-face member assessments; mileage reimbursed) providing case management—manage a caseload, complete telephonic/in-person assessments, build and monitor care plans, coordinate services (including prior authorizations and provider follow-up), connect members to community resources, and document progress to support wellness outcomes.
Bilingual Healthcare Case Manager Requirements:
- Fluent bilingual Spanish/English (read, speak, write) — required
- 1+ year case management experience — required
- Bachelor’s degree required (social work or related field preferred)
- Proficient with Microsoft Word, Excel, Outlook
- Strong communication skills; able to multitask and work in a fast-paced environment
- Long-term care experience (preferred)
- Case management certificate (preferred)
