Description
Travel Physical Therapist (PT) – Home Health
Location: Marysville, CA
Service Area: Grass Valley, Yuba County, Butte County, and Sutter County
Shift: Days (08:00 AM - 04:30 PM)
Schedule: Monday – Friday
Responsibilities
- Evaluate patients and develop individualized treatment plans to improve mobility, function, and independence.
- Provide skilled physical therapy services in patients' homes.
- Assess patient progress and modify treatment plans as necessary.
- Educate patients and caregivers on therapeutic exercises, mobility techniques, and injury prevention.
- Complete accurate and timely clinical documentation.
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care.
- Maintain compliance with home health regulations and agency policies.
Required Qualifications
- Active Physical Therapist License required.
- Current AHA BLS Certification required.
- Must be at least 21 years of age.
- Doctor of Physical Therapy (DPT) degree required for candidates who graduated in 2016 or later.
- Valid driver's license and reliable transportation required.
Preferred Qualifications
- Minimum 1 year of Home Health Physical Therapy experience preferred.
- New graduates are welcome to apply and will receive training.
