Required Qualifications

• Minimum Experience: 1 year required
• Home Health Experience: Required
• OASIS Training and Experience: Required
• Infusion Experience: Required
• Skilled Home Care Experience: Required
  • Private duty nursing experience alone will not be accepted
• IV Skills: Required
• Wound Care Experience: Required
• Valid Driver’s License: Required
• Reliable Personal Vehicle: Required for home visits

Required Certification

• BCLS (Basic Cardiac Life Support)
• COVID-19 Vaccine Required - Medical/Religious Exemptions Only
• Flu Vaccine Required - Medical/Religious Exemptions Only

Clinical Skills Required

Candidates must be experienced and comfortable performing:

• Infusion therapy
• IV skills
• Wound care
• Cardiopulmonary assessments
• Basic wound care
• ABI compression dressings
• Wound VAC dressing changes
• PleurX/Asept Drain care
• Foley catheter changes and maintenance
• Staple and suture removal
• Glucometer use
• Pro-time machine use
• SQ and IM injections
• Venipuncture and lab draws
• PICC line care and dressing changes
• Midline catheter care and dressing changes
• SQ port access and de-access

Patient Population & Diagnoses

General Medical Diagnoses

• General medical patients requiring skilled home care services

High-Risk Diagnoses

• Heart Failure (HF)
• Chronic Obstructive Pulmonary Disease (COPD)
• Myocardial Infarction (MI)
• Pneumonia
• Stroke

Additional Patient Types

• Post-operative patients
• Skilled Home Care patients covered by:
  • Medicare
  • Medicare Advantage
  • Commercial Insurance Plans

Schedule & Work Environment

Visit Expectations

• Home skilled visits: 6–8 patients per day

Schedule Options

Work hours may vary based on HomeCare office needs:

• Monday–Friday: 8:00 AM – 5:00 PM
• Monday–Friday: 9:00 AM – 6:00 PM
• Thursday–Monday shifts
• Friday–Tuesday shifts
• Alternating weekend coverage required

Scheduling

• Visits and work hours are self-scheduled based on HomeCare office needs.

Equipment & Documentation Systems

The following equipment and systems will be provided/used:

Dress Code

• Any solid-color scrubs

Additional Requirements

• Must be COVID-19 vaccinated or have an approved exemption form on file.
• Must comply with all HomeCare office scheduling and patient care requirements.
• Must be comfortable working independently in patients’ homes.
