Required Skills

• Active LMSW (Licensed Master Social Worker)
• Screen certification (Screen Cert)
• Proficiency with EPIC electronic medical record (EMR), with prior hands-on experience
• Strong assessment, crisis intervention, and care planning skills
• Ability to conduct psychosocial evaluations and develop appropriate interventions
• Excellent communication and collaboration skills with patients, families, and interdisciplinary teams
• Strong documentation skills and attention to detail within an EMR
• Ability to manage a diverse caseload and prioritize in a fast-paced environment
• Sound professional judgment, ethics, and adherence to regulatory and organizational standards

Nice to Have Skills

• Experience in both medical social work and behavioral health settings
• Familiarity with community resources, social service agencies, and entitlement programs
• Experience with discharge planning, care transitions, and complex case management
• Background in patient/family education, counseling, or group facilitation
• Bilingual or multilingual communication skills

Preferred Education and Experience

• Master’s degree in Social Work (MSW) from an accredited program
• At least 2 years of post-master’s experience, preferably in a medical and/or behavioral health setting
• Previous experience working in a hospital, clinic, or similar healthcare environment

Other Requirements

• LMSW and Screen certification required
• Experience using EPIC EMR required
• Availability to work a 9:00 a.m.–5:00 p.m. schedule
• Ability to provide some weekend coverage
