Physical Therapist (Outpatient) | Ukiah, CA | Days | 13-Week Contract

Position Details
• Position: Physical Therapist (PT)
• Specialty: Outpatient Therapy
• Start Date: July 6, 2026
• Schedule: Monday–Friday
• Hours: 8:00 AM–5:00 PM or 8:00 AM–4:30 PM (depending on lunch schedule)
• Weekly Hours: 40
• Call Requirement: None
• Floating: Not required

Responsibilities
• Evaluate and treat outpatient physical therapy patients
• Develop individualized treatment plans
• Restore and improve physical function and mobility
• Collaborate with interdisciplinary healthcare teams
• Educate patients and families on rehabilitation goals and home exercise programs
• Document patient progress and treatment outcomes

Required Qualifications
• Active California Physical Therapy License
• CPR or BLS Certification
• Physical Therapy degree from an accredited program
• Doctor of Physical Therapy (DPT) required for therapists licensed on or after January 2016

Preferred Qualifications
• Minimum 2 years of Physical Therapy experience
• Previous outpatient rehabilitation experience
