Position Overview
We are seeking a skilled and experienced Registered Nurse (RN) to provide high-quality home health care services to patients in the South Charlotte, East Charlotte, and Union County areas. This role focuses on delivering individualized patient care, completing OASIS documentation, and supporting patients in a home setting. The ideal candidate has prior home health experience, is proficient in HomeCare HomeBase, and is able to work independently.
Schedule & Work Details
- Location: South Charlotte, East Charlotte, and Union County
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours
- On-call/Weekend Requirement: None
- Productivity Expectation: 30 points per week
- Pay Rate: $51.00 to $53.00 per hour
- Mileage Reimbursement: Yes, between patient visits from first patient to last patient
- Dress Code: Business casual or scrubs
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Perform comprehensive patient assessments and ongoing evaluations
- Complete accurate and timely OASIS documentation
- Utilize HomeCare HomeBase for documentation and care coordination
- Develop and implement individualized care plans based on patient needs
- Educate patients and families on disease management, medications, and care plans
- Maintain clear and consistent communication with physicians and interdisciplinary team members
- Ensure compliance with agency policies and regulatory standards
Required Qualifications
- Active North Carolina Registered Nurse (RN) license
- Prior home health experience
- Experience with HomeCare HomeBase
- Strong clinical assessment and documentation skills
- Ability to manage caseload independently and efficiently
Preferred Qualifications
- Strong organizational and time management skills
- Excellent communication and patient education abilities
- Ability to work autonomously in a home-based setting
- Detail-oriented with strong critical thinking skills
