Position Overview
We are seeking an experienced Registered Nurse (RN) to provide comprehensive home health services to patients in Kent and Queen Annes Counties. This role focuses on managing a patient caseload, completing OASIS documentation, and delivering high-quality, patient-centered care. The ideal candidate has prior home care experience, strong clinical judgment, and the ability to work independently in a field-based setting.

Schedule & Work Details

• Location: Kent and Queen Annes Counties
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 40 hours
• On-call/Weekend Requirement: 1 weekend every 4 weeks
• Pay Rate: $51.00 to $53.00 per hour
• Mileage Reimbursement: Yes, between patient visits from first patient to last patient
• Dress Code: Business casual or scrubs of any color

Key Responsibilities

• Complete OASIS admissions, recertifications, resumptions of care, and discharges
• Perform routine revisits (RV) and provide ongoing patient care
• Manage a caseload of home health patients and coordinate care plans
• Conduct comprehensive patient assessments and document findings accurately
• Develop and update individualized care plans in collaboration with physicians and care teams
• Provide education to patients and families on treatment plans, medications, and disease management
• Ensure timely and compliant documentation in accordance with regulatory standards
• Communicate effectively with interdisciplinary team members to ensure continuity of care

Required Qualifications

• Active Registered Nurse (RN) license
• Previous home health or home care experience
• Strong knowledge of OASIS documentation
• Ability to manage a patient caseload independently
• Excellent clinical assessment and documentation skills

Preferred Qualifications

• Experience with HomeCare HomeBase (HCHB)
• Strong organizational and time management skills
• Excellent communication and interpersonal abilities
• Ability to work autonomously in a field-based environment
• Detail-oriented with strong critical thinking skills
