Overview

We’re seeking an experienced Registered Nurse (RN) to lead our home health clinical team as Director of Patient Care Services. In this role, you’ll drive high-quality patient care, ensure regulatory compliance, and support a collaborative, patient-centered environment.

Responsibilities

• Lead and supervise all home health clinical operations and staff
• Ensure quality patient care in accordance with physician orders and care plans
• Maintain compliance with Medicare, Medi-Cal, and state regulations
• Oversee staff hiring, training, performance, and scheduling
• Drive performance improvement and ensure accurate clinical documentation
• Coordinate interdisciplinary care and serve as a key liaison with physicians
• Manage patient admissions, discharges, and overall care delivery

Qualifications

• Active RN license
• 3+ years in home health or healthcare (leadership experience preferred)
• Strong knowledge of Medicare/Medi-Cal regulations
• Excellent leadership, communication, and organizational skills
• Experience with EMR systems and Microsoft Office

Salary range $140,000 to $150,000
