Travel Physical Therapist (PT) – Home Health Mayfield, KY
Coverage Area: Graves County & Calloway County, Kentucky
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Weekly Hour Guarantee: 40 Hours
On-Call/Weekend Requirement: None
Mileage Reimbursement: Yes, between patient visits (first patient to last patient)
 

Position Summary

The Physical Therapist will be responsible for evaluating patients, developing individualized treatment plans, and implementing therapeutic interventions designed to improve mobility, function, and overall quality of life.

Key Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Assess patient functional status, mobility limitations, strength, balance, and rehabilitation needs.
• Develop individualized treatment plans based on physician orders and patient goals.
• Implement evidence-based therapeutic interventions to improve patient outcomes.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients, families, and caregivers on exercises, safety measures, mobility techniques, and home programs.
• Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate care.
• Participate in interdisciplinary team meetings as required.
• Maintain compliance with Medicare, state, and agency guidelines.

Required Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the state of Kentucky or eligibility to obtain licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to travel throughout the assigned coverage area.

Preferred Qualifications

• Previous Home Health Physical Therapy experience.
• Experience conducting patient evaluations in home-based settings.
• Knowledge of Medicare and home health documentation requirements.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and patient education abilities.
