Description

Pride health is hiring a Registered Nurse for our client's Hospice program in Cordova, TN 38018.

 

Contract Duration: 3+ months (13 Weeks) Possible extension and potential for conversion into a permanent role.

Schedule: Monday – Friday, 8:00 AM – 4:30 PM

On-Call Requirement: None

Weekend Requirement: None

Coverage Area: Shelby, Fayette, and Tipton Counties (Memphis Metro Area)

EMR System: HomeCare HomeBase (HCHB)

 

Compensation & Benefits:

Local Pay Package: (Applicable to candidates whose primary residence is within a 50-mile radius of Cordova, TN)

  • $50.00 – $57.00 per hour (W-2)
  • Mileage Reimbursement: $0.49 per mile (Milage will be reimbursed between visits, from first patient to last patient.)

Travel Pay Package: (Applicable to candidates maintaining a primary residence outside a 50-mile radius of Cordova, TN)

  • $2,400.00 – $2,550.00 per week (Includes taxable & nontaxable parts)
  • Mileage Reimbursement: $0.49 per mile (Milage will be reimbursed between visits, from first patient to last patient.)

Position Overview:

  • The RN will function as a case manager, coordinating and delivering hospice services while collaborating with interdisciplinary team members to ensure patients receive comprehensive, patient-centered care.
  • This role requires strong hospice experience, proficiency with HomeCare HomeBase (HCHB), and a commitment to delivering exceptional care that supports patients and families during critical stages of illness.

Key Responsibilities:

  • Manage a caseload of hospice patients across assigned coverage territories.
  • Perform comprehensive nursing assessments and develop individualized plans of care.
  • Coordinate and oversee hospice services in collaboration with physicians, social workers, chaplains, aides, and other interdisciplinary team members.
  • Conduct routine skilled nursing visits and evaluate patient status.
  • Complete hospice admissions and start-of-care assessments as needed.
  • Respond to and perform death/pronouncement visits in accordance with agency policies.
  • Prepare documentation and clinical updates for Interdisciplinary Team (IDT) meetings.
  • Monitor symptom management, pain control, and medication effectiveness.
  • Educate patients, caregivers, and family members regarding disease progression, hospice philosophy, and care plans.
  • Maintain accurate and timely clinical documentation in HomeCare HomeBase (HCHB).
  • Ensure compliance with Medicare, hospice regulations, and agency standards.
  • Travel throughout assigned territories to provide patient care.

Required Qualifications:

  • Active Registered Nurse (RN) license in Tennessee or Compact State License.
  • Associate Degree in Nursing (ADN) or higher.
  • Minimum of one year recent hospice nursing experience.
  • Proficiency with HomeCare HomeBase (HCHB) required.
  • Experience with hospice case management, admissions, and end-of-life care.
  • Strong clinical assessment and care coordination skills.
  • Valid driver's license, reliable transportation, and auto insurance.

  • Previous experience with hospice admissions and death visits.
  • Strong communication and interpersonal skills.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
