Pride health is hiring a Registered Nurse for our client's Home Health program in Cordova, TN 38018.

Contract Duration: 3+ months (13 Weeks) Possible extension and potential for conversion into a permanent role.

EMR System: HomeCare HomeBase (HCHB)

Compensation & Benefits:

Local Pay Package:

• $50.00 – $58.00 per hour (W-2)
• Mileage Reimbursement: $0.49 per mile (Milage will be reimbursed between visits, from first patient to last patient.)

Travel Pay Package:

• $2,400.00 – $2,550.00 per week (Includes taxable & nontaxable parts)
• Mileage Reimbursement: $0.49 per mile (Milage will be reimbursed between visits, from first patient to last patient.)

Key Responsibilities:

• Provide skilled home health nursing care through comprehensive patient assessments, OASIS documentation, care plan implementation, medication management, patient/caregiver education, and coordination with the interdisciplinary team across Shelby County, Fayette, and Tipton.
• Manage a weekly productivity target of 30 points while traveling between patient homes, maintaining accurate and timely documentation, participating in the on-call rotation every 4 weeks as needed, and ensuring compliance with home health regulations and quality standards.

Required Qualifications:

• Active Registered Nurse (RN) license in Tennessee or Compact State License.
• Associate Degree in Nursing (ADN) or higher.
• Proficiency with HomeCare HomeBase (HCHB) required.
• Strong clinical assessment and care coordination skills.
• Valid driver's license, reliable transportation, and auto insurance.

Schedule details (hours per day/week): 40

Productivity expectations: 30 points a week

On Call/Weekend Requirement: possible on call rotation q 4 weeks
Coverage Areas: shelby county art of fayette and tipton

Shift schedule (days, start & end times): 8:00 AM to 04:30 PM
