Description
Pride health is hiring a Registered Nurse for our client's Home Health/Hospice program in Corpus Christi, TX 78411.
Contract Duration: 3+ months (13 Weeks) Possible extension and potential for conversion into a permanent role.

Schedule: Day Shift
On-Call Requirement: Preferred 1 week every 6 weeks
Compensation & Benefits:
Local Pay Package: (Applicable to candidates whose primary residence is within a 50-mile radius of Corpus Christi, TX)
- $50.00 – $57.00 per hour (W-2)
- Mileage Reimbursement: $0.48 per mile (Mileage will be reimbursed between visits, from first patient to last patient.)
Travel Pay Package: (Applicable to candidates maintaining a primary residence outside a 50-mile radius of Corpus Christi, TX)
- $2,400.00 – $2,550.00 per week (Includes taxable & nontaxable parts)
- Mileage Reimbursement: $0.48 per mile (Mileage will be reimbursed between visits, from first patient to last patient.)

Position Overview:
- We are seeking an experienced Home Health/Hospice Registered Nurse to deliver high-quality, patient-centered care in the field. The ideal candidate will have strong clinical assessment skills, experience managing a home health caseload, and proficiency with OASIS documentation.
Key Responsibilities:
- Conduct comprehensive patient assessments in home and hospice settings.
- Develop, implement, and update individualized Plans of Care (POC).
- Complete accurate OASIS assessments and clinical documentation.
- Coordinate care with physicians, therapists, social workers, and interdisciplinary teams.
- Educate patients and caregivers regarding disease management, medications, and treatment plans.
- Monitor patient progress and adjust interventions as needed.
- Ensure compliance with agency, state, and federal regulations.
- Manage a productivity expectation of 30 points per week.
- Maintain timely and accurate documentation within the EMR system.
- Travel throughout assigned territories to provide direct patient care.
Required Qualifications:
- Active Texas Registered Nurse (RN) License
- Bachelor's degree in nursing or related field
- Recent Home Health Field RN experience
- Strong clinical assessment and case management skills
- Valid driver's license, reliable transportation, and auto insurance
- Ability to work independently in the field
Preferred Qualifications:
- Hospice nursing experience
- OASIS proficiency
- Homecare Homebase (HCHB) experience
- Dependable, organized, and self-motivated
- Strong communication and patient education skills
Ideal candidates are experienced Home Health RNs who are comfortable working independently in the field, proficient with OASIS documentation, and passionate about delivering exceptional patient care.
