Position Summary:
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our healthcare team. The Social Worker will provide psychosocial assessments, discharge planning, care coordination, and support services to patients and their families in an acute care hospital setting. The ideal candidate will have experience working with inpatient medicine, surgery (Med/Surg), and rehabilitation patients.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop and implement individualized discharge plans to ensure safe transitions of care.
• Coordinate with physicians, nurses, case managers, rehabilitation staff, and community agencies.
• Assist patients with accessing community resources, financial assistance programs, and support services.
• Provide crisis intervention, counseling, and emotional support to patients and families.
• Advocate for patients' needs and assist with complex care planning.
• Maintain accurate and timely documentation in accordance with hospital policies and regulatory requirements.
• Participate in interdisciplinary team meetings and patient care conferences.

Requirements:

• Master's Degree in Social Work (MSW) required.
• Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED) required.
• Minimum one (1) year of recent hospital-based social work experience required.
• Experience in Inpatient Medicine, Surgery (Med/Surg), and/or Rehabilitation settings preferred.
• Strong knowledge of discharge planning, care coordination, and community resources.
