Description

Assignment Details

Duration: 13 weeks
Schedule: Monday–Friday
Shift: 8:00 AM – 4:30 PM

Requirements (HARD STOPS)

Active California RN license (or pending with activation by start date)
Minimum 2 years Acute Care Case Management / Discharge Planning experience
Hospital experience REQUIRED (Behavioral health experience does not qualify)
Epic experience required
Travel experience required (no first‑time travelers)

Responsibilities

Perform comprehensive patient assessments and care coordination
Lead discharge planning for acute care patients
Conduct utilization review and ensure appropriate level of care
Facilitate family meetings and care plan discussions
Arrange post‑acute services including SNF, DME, and transportation
Maintain accurate documentation using Epic EMR
