Physical Therapist Assistant - Inpatient Rehab

Location: Saint Cloud, Minnesota
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Work schedule: 5 days per week, 8-hour shifts (8:00 AM - 4:30 PM)
• Every 4th weekend required (Saturday & Sunday)
• No on-call duties
• Charting system: EPIC (experience preferred)
• Scrub color: Royal Blue
• Must live more than 25 miles from the facility (no local candidates accepted)

Required Qualifications

• Valid Physical Therapy Assistant license in Minnesota
• Minimum 1 year of Physical Therapy Assistant experience within the last year
• Basic Life Support (BLS) certification

Background and Compliance Requirements

• Criminal background check within 1 year of start, including:

  • 7-year search
  • Social Security trace
  • County and nationwide checks
  • Sex offender registry
  • Education verification (diploma acceptable in lieu of third-party verification)
  • International background checks if candidate has lived/worked outside the U.S.
• Minnesota DHS NetStudy Fingerprint clearance required before start (cannot start with fingerprints pending unless a “more time needed” letter is approved by the facility)
• Must complete Minnesota DHS Net Study Questionnaire immediately after signing contract
• Two references from recent employers within 2 years required
• Skills checklist required

Health and Immunization Requirements

• Physical exam within 1 year of start
• Fit testing for N95 masks required (Halyard Fluidshield or 3M 1860)
• Tetanus/Diphtheria/Tdap vaccination required (no religious declinations accepted)
• Tuberculosis screening per Minnesota state guidelines (2-step PPD or equivalent testing within 90 days of start)
• Measles, Mumps, Rubella (MMR) immunity proof required (positive titer or 2 vaccines; must provide documentation or lab testing within 48 hours of start)
• Varicella immunity proof required (positive titer or 2 vaccines; same timing as MMR)
• Hepatitis B vaccination, positive titer, or declination accepted
• Seasonal flu vaccination or declination required annually (decliners must wear a mask)
• Drug testing: 9-panel test within 1 year of start (instant tests acceptable)

Additional Facility Requirements

• Candidate must sign confidentiality agreement prior to start
• Photo for badge required (grey background, no selfies, JPG format)
• Dress code must be reviewed prior to start
• Education modules completed during orientation (average 16 hours)
• Candidates must return badges to manager on last day of assignment
