Pride health is actively hiring a Physical Therapist to join our client's home health program in a growing care team serving patients in Hagerstown, MD, 21740.

Weekly Hour Guarantee: 40 hours
 

Productivity Expectations (points): 30 points
 

Coverage Areas: Washington County, MD
 

Shift schedule (days, start & end times): 8am-4:30 pm Monday-Friday. Mandatory weekly case conference 8:30-9:30am on Thursdays.
 

On Call/Weekend Requirement: No

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes ($0.49/Mile)

💰 Compensation & Benefits:

• Local Pay range: $60.00 to $67.00 an Hour on W-2
• Travel Package (Applicable to candidates maintaining a primary residence outside a 50-mile radius of Point Pleasant, WV): $2,800 – $3,000 per week (Include both taxable and non-taxable parts)

Contract Duration: 18 Weeks (4+Months, Possible extension and potential for conversion into a permanent role.)

What are the top responsibilities for this position? (Please be as detailed as possible): Collaboration with other team members including nurses, physical therapy assistants, occupational therapy, and home health aide about patients including changes, concerns, and plan of care. Develop and follow through with plan of care developed between provider and physical therapist. 

Must have qualifications/experience: Home health experience preferred but can have 1 plus years of hospital, SNF, or outpatient experience.
