Registered Nurse (RN) to join our Home Health team. This position focuses on conducting admissions, providing case management services, and overseeing LPN clinical activities while delivering high-quality patient-centered care in the home setting.

Job Title: Home Health Registered Nurse (RN) – Admissions & Case Management

Location: Richland County, SC

Schedule & Productivity

• Full-Time, 40 hours per week
• Productivity Expectation: 30 points per week
• Flexible scheduling with autonomy in managing daily patient visits
• Office Hours: Monday–Friday, 8:00 AM – 4:30 PM
  
  Key Responsibilities
• Complete home health admissions and comprehensive patient assessments
• Develop, implement, and manage individualized care plans
• Perform and document OASIS assessments accurately and timely
• Provide ongoing case management for assigned patients
• Supervise and support LPN staff in delivering patient care
• Evaluate and monitor patient conditions, identifying changes in health status
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members
• Educate patients and families on disease management, medications, and treatment plans
• Ensure compliance with agency policies and regulatory requirements
  
  Required Qualifications
• Active Registered Nurse (RN) license
• Current CPR Certification
• Home Health experience
• OASIS experience
• Valid driver's license and reliable transportation
  
  Preferred Qualifications
• Homecare Homebase (HCHB) experience
• Strong wound assessment and wound care management skills
• Excellent communication and time management abilities
