Description
Registered Nurse (RN) to join our Home Health team. This position focuses on conducting admissions, providing case management services, and overseeing LPN clinical activities while delivering high-quality patient-centered care in the home setting.
Job Title: Home Health Registered Nurse (RN) – Admissions & Case Management
Location: Richland County, SC
Schedule & Productivity
- Full-Time, 40 hours per week
- Productivity Expectation: 30 points per week
- Flexible scheduling with autonomy in managing daily patient visits
- Office Hours: Monday–Friday, 8:00 AM – 4:30 PM
Key Responsibilities - Complete home health admissions and comprehensive patient assessments
- Develop, implement, and manage individualized care plans
- Perform and document OASIS assessments accurately and timely
- Provide ongoing case management for assigned patients
- Supervise and support LPN staff in delivering patient care
- Evaluate and monitor patient conditions, identifying changes in health status
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members
- Educate patients and families on disease management, medications, and treatment plans
- Ensure compliance with agency policies and regulatory requirements
Required Qualifications - Active Registered Nurse (RN) license
- Current CPR Certification
- Home Health experience
- OASIS experience
- Valid driver's license and reliable transportation
Preferred Qualifications - Homecare Homebase (HCHB) experience
- Strong wound assessment and wound care management skills
- Excellent communication and time management abilities
