Pay Rate: $40.00/hour

Assignment Dates: 06/22/2026 – 06/27/2027

Job Type: Temporary Contract, Temp-to-Perm (opportunity to extend or convert to permanent employment based on business needs)

Schedule: Full-Time or PRN Available (please indicate preference)

Shift: 6:00 PM – 6:00 AM

 

Position Summary:

The Licensed Practical Nurse (LPN) is responsible for administering medications and performing treatments on an assigned unit as ordered by the physician and within the scope of licensure. This role supports resident-centered care through clinical assessments, treatment administration, documentation, communication, and participation in quality improvement initiatives.

 

Key Responsibilities:

  • Administer medications in various forms, including oral, enteral, parenteral, and suppository medications
  • Perform treatments including:
    • Sterile and aseptic wound cleaning and dressing changes
    • Postural drainage and percussion
    • Tracheostomy care
    • Gastrostomy and jejunostomy care
    • Catheter care
  • Observe and report the effectiveness of medications and treatments
  • Ensure adequate medication and medical supply inventory is maintained
  • Receive and communicate resident medical, social, functional, and care-related information
  • Participate in shift change reports
  • Communicate effectively with residents, families, and interdisciplinary team members
  • Complete initial nursing assessments for newly admitted residents
  • Assist with development of:
    • Minimum Data Set (MDS) assessments
    • Quarterly reviews
    • Resident care plans
  • Maintain accurate clinical and administrative documentation regarding diagnoses, treatments, observations, and resident progress
  • Participate in the Quality Assurance and Performance Improvement (QAPI) program
  • Attend and actively participate in staff meetings

 

Required Qualifications:

  • Current Licensed Practical Nurse (LPN) license/certification
  • CPR/BLS certification
  • Ability to comply with all nursing regulations and standards of practice
  • Ability to listen, observe, assess, and respond appropriately to medical emergencies
  • Ability to evaluate resident conditions and make independent clinical decisions within scope of practice
  • Ability to respond professionally to medical issues and inquiries
  • Ability to establish and maintain effective working relationships with residents, families, and healthcare staff
  • Strong oral and written communication skills
  • Strong interpersonal skills

 

Required Knowledge:

  • Gerontological and geriatric nursing theory and practice
  • Effects of the aging process
  • Team nursing practices and procedures
  • Treatment planning and implementation for resident care
  • Pharmacology principles and medication administration
  • Federal, state, and agency laws, rules, and regulations governing practical nursing
  • Medical supply and medication security, control, and dispensing procedures

Required Skills:

  • Observing and reporting resident behavior, condition, and progress
  • Applying and interpreting state and agency regulations related to geriatric nursing
  • Assisting in the development and implementation of patient care plans
  • Clinical documentation and recordkeeping
  • Collaboration within an interdisciplinary healthcare team
