Description

Required Certifications:

BLS, WI PT License in hand. NPI # required. If the candidate does not have one, they need to start the process of applying for a NPI # in order to start.
(Aspirus requires AHA or ARC certifications)

Required Education: Master's Degree in Physical Therapy.

Minimum Experience: 2 years of home Health experience highly preferred, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis. Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.
