Position Details

Hours per Shift: 8 Hours
Weekly Hours: 40 Hours
Experience Required: 5+ Years
Certification Required: Registered Dietitian (RD)

Key Responsibilities

Perform nutrition assessments and screenings for high-risk patients.
Develop and implement individualized nutrition treatment plans in collaboration with the interdisciplinary team, patients, and families.
Monitor patient progress and modify nutrition interventions as needed.
Create Nutrition Care Plans and MDS plans for long-term care patients.
Provide nutrition recommendations and consultations to physicians.
Maintain accurate and timely documentation in patient medical records.
Educate patients and caregivers on nutrition-related health needs and dietary modifications.
Participate in interdisciplinary meetings, care conferences, unit rounds, and continuum of care meetings.
Contribute to quality improvement initiatives and development of clinical nutrition standards of care.
Collaborate with healthcare teams to enhance patient outcomes and nutritional care services.
