Description
Licensed Practical Nurse (LPN) – Long-Term Care / Nursing Home
Location: Brooklyn, New York
Duration:13 Weeks
Available Shifts
- Days: 7:00 AM – 3:15 PM (Every Other Weekend)
- Evenings: 3:00 PM – 11:15 PM (Every Other Weekend)
- Nights: 11:00 PM – 7:15 AM (Every Other Weekend)
Responsibilities
- Provide direct nursing care to residents in a Long-Term Care environment
- Monitor patient conditions and report significant changes
- Assist in developing and updating individualized care plans
- Administer medications and treatments as prescribed
- Maintain accurate and timely documentation
- Collaborate with physicians, RNs, care managers, and interdisciplinary teams
- Support discharge planning and post-care coordination
- Ensure compliance with facility policies, procedures, and safety standards
Requirements
- Active New York State LPN License
- Graduate of an accredited Practical Nursing program
- Minimum 1 year of clinical experience preferred
- Long-Term Care (LTC) / Nursing Home experience required
- Strong communication and patient care skills
- Professional demeanor and reliability
- EPIC experience preferred
