Home Health RN Highlights

17-week contract assignment
Strong potential for permanent hire based on performance
Full-time schedule
Shift: 8:00 AM – 4:30 PM
One Saturday per month required
Supporting patients throughout Wayne, Oakland, and Macomb Counties
Approximately 6 patient visits per day
Average daily travel of 40–60 miles
Orientation includes classroom and field training with a preceptor

Home Health RN Responsibilities

Provide direct patient care in the home setting
Complete patient assessments and develop care plans
Coordinate care with physicians and interdisciplinary healthcare teams
Identify the need for additional services such as physical therapy, occupational therapy, speech therapy, nutrition, and social work
Educate patients and families on treatment plans and disease management
Maintain accurate and timely documentation

Home Health RN Requirements

Active Michigan RN license
BLS certification through the American Heart Association
Minimum 1 year of Home Health RN experience required
Graduate of an accredited Registered Nursing program
Current valid driver's license with satisfactory driving record
Reliable transportation and telephone required
Strong communication, customer service, and interpersonal skills
