3x12 hrs, Days, Nights

Available Shift

6:00 PM – 6:00 AM

Candidates may be scheduled for:

* 3 twelve-hour shifts one week
* 4 twelve-hour shifts the following week

Additional shifts and overtime opportunities may be available.

Key Responsibilities

* Assist clients with activities of daily living (ADLs), including feeding, bathing, dressing, grooming, and personal care.
* Monitor and document vital signs, height, weight, and fluid intake/output.
* Observe and report changes in patient condition.
* Provide tracheostomy (trach) care, including:
   * Suctioning
   * Inner cannula changes
   * Strap changes
   * Humidification/HME support
* Assist with G-tube and suprapubic catheter care.
* Administer nebulizer treatments.
* Perform blood glucose monitoring and assist with insulin administration within scope of practice.
* Administer medications as prescribed.
* Maintain accurate patient records and care documentation.
* Collaborate effectively with caregivers, agency staff, and healthcare professionals.

Minimum Qualifications

Required

* Active Licensed Practical Nurse (LPN) License.
* Current CPR Certification (Red Cross preferred).
* Valid Driver’s License.
* Ability to pass required background checks, fingerprinting, and safety clearances.
* Ability to work 12-hour shifts, including nights, weekends, and holidays.

Preferred

* Previous experience in:
   * Home Health
   * Direct Care
   * Long-Term Care
   * Skilled Nursing
   * Medically Complex Patient Care
* Experience with:
   * Tracheostomy Care
   * Suctioning
   * G-Tube Care
   * Catheter Care
   * Nebulizer Treatments
   * Medication Administration
