Description

Social Worker – Case Management (Discharge Planning)

Location Requirement

Must live 50 miles or more away from the facility

Shift

8:30 AM – 5:00 PM
Monday to Friday (M–F)

Required Experience

Minimum 1 year hospital Case Management experience required
Experience in Discharge Planning is required
Knowledge of Cerner EMR required

Required Licensure & Certifications

Active DC Social Work License
LICSW (Licensed Independent Clinical Social Worker) required

Required Skills

Hospital case management
Discharge planning coordination
Patient assessment and care coordination
Collaboration with physicians, nurses, and multidisciplinary teams
Insurance and utilization review understanding
Resource planning and placement (SNF, rehab, home health)
Strong documentation and communication skills
EMR experience (Cerner)

Ideal Candidate Profile

A strong candidate will have:

1+ year hospital-based case management experience
Direct discharge planning experience in acute care hospital
Active DC LICSW license
Strong Cerner EMR experience
Experience coordinating post-acute care services
Strong communication and organizational skills
