Travel RN – Case Manager / Discharge Planner
Start Date: ASAP
Specialty: Case Management
First-Time Travelers: Not Accepted
Requirements
- Minimum 2 years of acute care hospital Case Management experience
- Strong discharge planning and care coordination skills
- Experience collaborating with physicians, nursing staff, social services, and post-acute providers
- California RN license preferred
- Pending California license accepted
Assignment Details
- Patient Ratio: Approximately 1:35
- Will be responsible for discharge planning and utilization management
- On weekends, may be assigned to oversee up to two inpatient units
- Must be comfortable managing a high-volume caseload and prioritizing discharges efficiently
Schedule
- Every other weekend required
- Must be available for holiday coverage
- Requested time off limited to 7 days maximum
