Description
Shit timings:
Day 5x8-Hour (08:00 - 16:30) (Every other weekend, Must be available for holiday coverage)
Work Experience:
2-year acute hospital case management experience required.
Performing role of care coordination and discharge planning.
Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, POLST.
Performing both roles of Care Coordination and AIDA, PASRR.
Education:
Master's of Social Worker
License:
CA State License preferred.
