Description
- Position: RN-Home Health
- Facility: North Country Home Health & Hospice Agency
- Location: 536 Cottage St, Littleton, NH 03561
- Contract Duration: 13 weeks (with possible extension)
- Shift: (5x8) Day Shift (8:00 am – 5:00 pm) (M-F)

Registered Nurse (RN) – Home Health & Hospice
Position Overview
Join a compassionate and patient-centered Home Health & Hospice team where you'll have the opportunity to provide individualized care to patients across all stages of life in the comfort of their homes. This role offers a high degree of autonomy, flexibility, and professional growth while working alongside a collaborative, multidisciplinary healthcare team.
As a Home Health RN, you will develop and implement personalized care plans, coordinate services, and deliver high-quality clinical care that promotes patient independence, comfort, and well-being. Strong clinical support, ongoing education, and certification opportunities are available to help advance your career.
Minimum Qualifications
- Active New Hampshire RN License (Compact License Accepted)
- Current AHA BLS Certification
- Minimum 18 months of recent Home Health experience
- Valid driver's license and current auto insurance
- Proficiency with electronic medical records (EMR)
- Ability to work independently with minimal supervision
- Strong communication, organizational, and critical thinking skills
Preferred Qualifications
- Previous Hospice experience
- 2–5 years of Medical-Surgical nursing experience
- Hospice skills competency documentation
