• Contract Duration: 13 weeks (with possible extension)  
• Shift: (5x8) Day Shift (8:00 am – 5:00 pm) (M-F)

Registered Nurse (RN) – Home Health & Hospice

Position Overview

As a Home Health RN, you will develop and implement personalized care plans, coordinate services, and deliver high-quality clinical care that promotes patient independence, comfort, and well-being. Minimum Qualifications

• Active New Hampshire RN License (Compact License Accepted)
• Current AHA BLS Certification
• Minimum 18 months of recent Home Health experience
• Valid driver's license and current auto insurance
• Proficiency with electronic medical records (EMR)
• Ability to work independently with minimal supervision
• Strong communication, organizational, and critical thinking skills

Preferred Qualifications

• Previous Hospice experience
• 2–5 years of Medical-Surgical nursing experience
• Hospice skills competency documentation
