Job Details:
Schedule: 8:00am-5:00pm
Location: Olympia, WA
Job Type: Contract
Contract Length: 6 months (possibility of extension)
Coverage Areas- Olympia and surround area
Pay Range: $52/hr- $56/hr
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
Key Responsibilities:
- Perform comprehensive patient evaluations and develop individualized plans of care
- Provide skilled physical therapy services in patients’ homes
- Conduct Start of Care (SOC), Resumption of Care (ROC), recertifications, and discharge visits
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on exercises, safety, and ongoing care
- Maintain timely and accurate clinical documentation in compliance with regulations
- Collaborate with physicians, nurses, and interdisciplinary team members
- Ensure compliance with Medicare guidelines and home health regulations
Required Qualifications:
- Degree in Physical Therapy (PT)
- Active Physical Therapist license (or eligibility) in the state of Connecticut
- Prior home health experience preferred
- Strong understanding of patient care, treatment planning, and documentation
- Excellent communication and organizational skills
- Ability to work independently and manage a flexible schedule
Preferred Qualifications
- Experience with OASIS documentation
- Familiarity with home health EMR systems (e.g., HCHB)
- Previous travel or contract experience
