<p><strong>Duties/Responsibilities LCSW shall include, but are not limited to: Licensed Clinical Social Worker</strong></p><ul><li>Accuracy in communications is critical to ensure timely, correct care is provided.</li><li>Periodically serves as clinician-of-the-day by being available for on-call during scheduled workdays for patient emergencies.</li><li>Advanced specialty knowledge, clinical skills, and expertise to assess patients and develop a list of actual and potential needs/diagnoses.</li><li>Responsible for gathering health history of patients through an interview of patient and/or family members.</li><li>Applies advanced knowledge, skills, and expertise in planning and implementation of patient’s care.</li><li>Maintain patient documentation in a prompt and comprehensive manner.</li><li>Ensures patient calls are returned and patient follow ups for referrals are handled promptly</li><li>Provide outstanding behavioral healthcare for clients of all ages by collaborating with primary health care providers, specialists, psychiatrists, dependency programs and other healthcare professionals</li></ul><p> </p>
