Position:Physical Therapist (PT)-SNF

Location:Seattle, WA 98126
Job Type: 13 Weeks Contract + possible extensions

Shift: Day 5x8-Hour (08:00 - 16:30)

Schedule: 5 days/week

Expected Hours: 40 hours per week

Note:Local Candidates are accepted for the job,Considered "local" within 50-75 mile radius.
Pay Range:$65-68/hrs. 

Job Overview

A Physical Therapist (PT) in a Skilled Nursing Facility (SNF) evaluates and treats patients with physical impairments, injuries, or mobility limitations. The PT develops individualized treatment plans to improve strength, balance, mobility, functional independence, and overall quality of life, helping patients achieve their highest level of function.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide therapeutic exercises, gait training, balance training, and mobility interventions.
• Assess and train patients in the use of assistive devices such as walkers, canes, and wheelchairs.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety precautions.
• Monitor patient progress and modify treatment plans as needed.
• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Participate in interdisciplinary care planning meetings.

Required Qualifications

• Minimum 1 year of experience as a Physical Therapist in SNF/LTC
• BLS/CPR
• Physical Therapist WA License
