Physical Therapist — Home Health | Flexible Full-Time, Part-Time & PRN Options in San Jose, CA area 

We are partnering exclusively with a five-star, clinician-led home health organization in the Bay Area that is looking for a Physical Therapist who believes great outcomes start with genuine relationships.

This isn’t a revolving-door caseload. It’s a role built for a clinician who wants to practice at the top of their license, own their day, and see the direct impact of their work — one patient at a time, right in their home.

Compensation & Schedule

• Salary:$130,000 – $140,000 annually
• Schedule:Monday – Friday, 9 am – 5 pm (Part time and PRN opportunities also available)
• Visits per day:Approximately 5
• Benefits:Comprehensive package including health, PTO, mileage reimbursement, and more

What We’re Looking For
Required:

• Active California Physical Therapist license (in good standing)
• Graduate degree in Physical Therapy (DPT preferred)
• Experience managing a patient caseload independently
• Strong communication and clinical decision-making skills
• Reliable transportation for home visits
• Home health experience

Preferred:

• Experience with Medicare/OASIS documentation
• Familiarity with home health EMR systems (e.g., WellSky/Kinnser)
• Experience working with geriatric or medically complex patients
