Description

Travel Registered Nurse (RN) – Hospice

Job Type: Travel Contract
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
On-Call Requirement: Required; shared rotation with hospice team members
Patient Volume: Approximately 7–8 patients per day (may vary based on census and patient needs)

Key Responsibilities

  • Conduct comprehensive nursing assessments and ongoing evaluations of hospice patients.
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
  • Manage and monitor patient symptoms, pain control, and overall comfort.
  • Perform medication reconciliation and coordinate medication refills as needed.
  • Communicate patient status changes, care needs, and recommendations to physicians and other healthcare providers.
  • Complete all required clinical documentation accurately and timely using Homecare Homebase (HCHB).
  • Participate in weekly Interdisciplinary Group (IDG) meetings and complete required IDG documentation.
  • Educate patients and caregivers regarding disease progression, symptom management, medications, and available hospice services.
  • Coordinate care with physicians, social workers, chaplains, aides, and other members of the hospice team.
  • Respond to on-call responsibilities according to established rotation schedules.
  • Maintain compliance with hospice regulations, agency policies, and clinical standards.

Required Qualifications

  • Active, unrestricted Tennessee Registered Nurse (RN) license or Compact State RN License.
  • Previous hospice nursing experience required.
  • Experience using Homecare Homebase (HCHB) documentation system required.
  • Current CPR certification.
  • Strong assessment, clinical decision-making, and care coordination skills.
  • Ability to work independently in the field.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple priorities and adapt to changing patient needs.
