Position Highlights

• 13-week travel assignment
• 36 hours per week (3x12-hour shifts)
• Rotating Days, Evenings, and Nights
• Weekend, holiday, and on-call rotation as needed
• Manage a resident-focused patient assignment in a long-term care environment
• Supportive interdisciplinary team including LPNs and CNAs
• Opportunity to provide comprehensive geriatric and wound care nursing

Responsibilities

• Deliver direct patient care to long-term care residents
• Perform nursing assessments and ongoing evaluations
• Develop, implement, and update individualized care plans
• Administer medications and treatments safely and accurately
• Provide wound care management
• Supervise and support LPNs and CNAs
• Maintain accurate and timely documentation
• Collaborate with physicians and interdisciplinary team members to ensure quality resident care

Patient Population

• Geriatric long-term care residents
• Wound care experience preferred
• No tracheostomy patients
• No acute care admissions within the facility

Requirements

✅ Active New York State RN License (required)
✅ Minimum 2 years of Long-Term Care RN experience
✅ Current BLS Certification through the American Heart Association (AHA)
✅ Strong assessment, communication, delegation, and documentation skills
✅ Experience with electronic medical records (EMR); Epic experience preferred
