Licensed Practical Nurse (LPN) — Long-Term Care Facility

FSN is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at a Long-Term Care Facility. In this role, you will provide direct patient care under the supervision of Registered Nurses (RNs) and physicians, supporting high-quality care while prioritizing patient safety, comfort, and well-being.

Commitment: 6-month assignment
Schedule: 40 hours/week, 8-hour shifts, 3 weekends per month
Shift hours: 9:45pm to 6:15am
Hourly Pay Rate: $39 with a 5% pay increase if extended after 6 months

Orientation & Education Requirement:
You will be expected to attend EVERY day of New Employee Orientation and Nursing Education, Monday–Friday, 8:00am–5:00pm, for approximately 2 weeks.

Key Responsibilities

• Provide direct nursing care, including:
  • Monitoring vital signs
  • Administering medications
  • Assisting with daily patient care needs
• Observe and report changes in patient conditions to the supervising RN or physician
• Assist with patient admissions, discharges, and transfers
• Perform basic wound care, dressing changes, and specimen collection
• Maintain accurate and timely documentation
• Educate patients and families on care plans, medications, and post-discharge instructions
• Collaborate with interdisciplinary teams to support patient outcomes
• Help maintain a clean, safe, and organized patient care environment

Qualifications

• Current, unencumbered state-issued LPN license
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) certification
• Strong clinical and patient care skills
• Excellent communication and interpersonal skills
