Description
Schedule: Day Shift | Monday–Friday | 8:00 AM – 4:30 PM (5×8s)
Department: Home Health
Required Qualifications
- Active LVN license in the state of practice
- BLS – Healthcare Provider certification (approved vendor per AH policy)
- Ability to work independently in a home health setting
Key Responsibilities
- Provide skilled nursing care to patients in their homes
- Monitor patient conditions and report changes to supervising clinicians
- Administer medications and treatments per care plan
- Educate patients and families on health management and care instructions
- Accurately document visits and patient progress
- Maintain compliance with home health policies and safety standards
