Description
Position Summary
We’re seeking an experienced LPN to support residents in a Long Term Care / Transitional Care environment. This is a fast-paced role requiring strong clinical judgment, comfort with higher patient ratios, and willingness to float between units as needed.
Schedule
- Shift: Nights (8-hour shifts)
- Schedule pattern: 5 shifts/week
- Weekend requirement: Every other weekend
- On-call: No
- Block scheduling: May be accommodated
- Assignment length: 13 weeks
Core Responsibilities
- Provide direct nursing care, assessments, and ongoing monitoring for residents in LTC/TCU settings
- Administer medications and treatments per orders and policy
- Perform and document IV-related care and wound care interventions
- Collaborate with interdisciplinary team members to support safe, high-quality patient care
- Serve in a charge/lead capacity when needed
- Float between Long Term Care and Transitional Care units based on staffing needs
Required Experience
- 2+ years recent Skilled Long Term Care / Transitional Care experience (required)
- 1+ year travel contract experience (required; first-time travelers not accepted)
- Charge experience (required)
- Comfortable with higher patient ratios (may vary by shift)
Required Skills (Must Have)
- IV administration/management
- Starting IVs
- IV pump operation
- Central line blood draws
- Central line dressing changes
- Wound care / wound vac
- Managing wound drains (JP/Hemovac)
Licensure & Certifications
- Active LPN license (pending license accepted)
- BLS (Basic Life Support)
Important Notes / Policies
- No travel pairs
- Previous permanent employees of the client system not accepted
- Time off: Limited (one request allowed); max 3 days during the assignment
- Floating is expected; refusal to float may impact assignment continuation/guaranteed hours
