Contract Home Health Physical Therapist (PT)
Location: East Portland, OR area + Clark County, WA (and surrounding areas)
Schedule: Days | 5x8s (approx. 08:00–16:30) | 40 hrs/week | 13-week assignment | Start ASAP
Position Overview
We’re seeking a Home Health Physical Therapist to provide 1:1, in-home patient care across a defined territory. Typical workload may include up to ~6 visits/day with a productivity expectation of ~30 points/day.
Key Requirements (Must Have)
- Active OR and WA PT licensure (dual-state required)
- 1+ year skilled home health experience
- OASIS experience
- Reliable personal transportation
Clinical / Workflow Details
- Setting: Home Health
- Charting: Epic (Remote Client) + Rover (mobile app) / OASIS
- Territory/Drive: Visits are grouped when possible; occasional days may include a larger radius (up to ~60 miles / 60–90 minutes total drive time)
- Mileage: Reimbursed (per IRS standard)
Schedule & Call/Weekend
- Weekends: Not required
- On-call: Not required
- Block schedule: Not available
- Guarantee/call-offs: Client may call off up to one shift per pay period (every 2 weeks)
Dress Code / Logistics
- Attire: Business casual (no jeans); scrubs allowed (no specific color)
- Parking: Free
