NOW HIRING: Home Health Registered Nurse

Location: Albemarle, NC

Hours: Monday - Friday, 8:00 AM - 5:00 PM

Pay: $56.00 per hour (Not Per Visit)

Scheduled Pay: Weekly

Mileage Reimbursement: $0.41 per mile

Counties covered: Stanly, parts of Davidson, and Montgomery

Benefits: Health, Dental, Vision, Life Insurance, and Short-Term Disability after 90 days

Home Health RN Case Manager Responsibilities

• Productivity Expectation: 5-7 visits per day and/or 6 points per day
• Conduct comprehensive patient assessments in the home setting and develop individualized care plans based on patient needs and physician orders.
• Perform skilled nursing services including medication administration, wound care, injections, disease management, and ongoing patient monitoring.
• Monitor patient conditions, identify changes in health status, and communicate updates to physicians and interdisciplinary care team members.
• Educate patients and caregivers regarding diagnoses, medications, treatment plans, disease prevention, and health maintenance strategies.
• Coordinate care with physicians, therapists, social workers, caregivers, and other healthcare professionals to ensure continuity of care.
• Manage medication orders, reviews, monitoring, discontinuation, and documentation in accordance with physician orders and agency policies.
• Complete accurate and timely clinical documentation within 24 hours of patient visits.
• Weekend on-call rotation required.

Home Health RN Case Manager Requirements

• Active, unrestricted North Carolina Registered Nurse License required
• Valid driver's license, insurance and reliable transportation required
• Wound care experience required
• Must have IV fusion and PICC care experience
• 2 years of Home Health experience required
• 2 years of OASIS experience required
• 2 years of Homecare Homebase (HCHB) experience required
