Description
🕒 Schedule: Monday–Friday | 8:00 AM – 4:00 PM
📋 Occasional Weekend Coverage (2–3 Days)
We are seeking an experienced Home Health Physical Therapist to join a leading hospice and home health organization in Vermont.
✅ Requirements:
• Active Physical Therapist License (VT or Obtain Upon Acceptance)
• Minimum 2 Years of PT Experience
• Home Health Experience Preferred
• OASIS Documentation Experience Required
• Ability to Work Independently in the Community Setting

• Conduct Comprehensive PT & OASIS Assessments
• Develop and Implement Individualized Treatment Plans
• Provide Skilled Home Health Physical Therapy Services
• Patient & Caregiver Education
• Care Coordination with Interdisciplinary Teams
• Supervise PTAs and Therapy Students
• Documentation in McKesson EMR
