Job Summary
WesleyLife is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join its Home Health team serving the Urbandale and Des Moines metro area. This is an excellent opportunity for LPNs looking to work independently while providing high-quality patient care in the home setting.
Whether your background is in Home Health, Skilled Nursing, Long-Term Care (LTC), Rehabilitation, or Post-Acute Care, we encourage you to apply.
Schedule
- Monday – Friday
- No weekends
- No on-call
- 5–6 patient visits per day
- Productivity expectation: 30–35 points per week
Responsibilities
- Provide skilled nursing care in patients' homes
- Complete patient assessments and ongoing monitoring
- Administer medications and treatments as ordered
- Educate patients and families regarding disease management and care plans
- Document patient visits accurately and timely
- Collaborate with physicians, nurses, therapists, and interdisciplinary care teams
- Monitor patient progress and communicate changes in condition
- Support patients in maintaining independence and quality of life
Requirements
Required
- Active LPN License
- Current BLS Certification (AHA)
- Reliable transportation and valid driver's license
- Strong communication and documentation skills
Preferred
- Home Health experience
- Skilled Nursing Facility (SNF) experience
- Long-Term Care (LTC) experience
- Rehabilitation experience
- WellSky EMR experience
Territory
- Urbandale
- Des Moines Metro Area
