Description

Job Title: Licensed Practical Nurse (LPN)

Duration: 11 Months

Location: MacClenny, FL (3 days a week, 12 hr. shifts)

Onsite/Hybrid/Remote: 3 Days onsite (7am-7:30pm - 30 min unpaid lunch) 

Interview: In Person Only

Rate is $40/hr on W2 (With no benefits)

I wanted to share a few important details before Proceeding:

12-hour shifts required
• Rotating weekends included
• No time off during the first 90 days 
• Mandatory 4-hour shift on Wednesdays
• Strict attendance (no late arrivals or early leaves) 
• Any emergency must be communicated and approved in advance


I just wanted to inform you I wanted to share a few important details about the role before your start date: 

• The position requires 12-hour shifts 

• Rotating weekend shifts are part of the schedule 

• During the first 90 days, no time off is permitted 

• Please note that 4-hour shifts on Wednesdays are mandatory. 

  • Candidates should be entering NEO with a clear understanding that they are not to miss any NEO (first 14 days) time

• Attendance is very important – no late arrivals or early departures 

• Any emergency must be communicated and approved in advance by the supervisor and scheduler If there is a real emergency, you must inform and get approval from your supervisor and scheduler in advance. Any attendance or schedule issues may lead to immediate removal from the assignment.

TYPE OF PATIENTS:  Two categories of patients are treated at the hospital; those civilly committed under Statute 394, who represent a small portion of the hospital's residents; and those forensically committed under Statute 916. The Civil portion of the hospital houses adult and elderly individuals who have been civilly committed to the hospital, and forensic residents who have been "stepped down" to the civil unit. The civil units are also known as Forensic Transition units. Forensic ward entry has security guards and a metal detector.  Most patients are ambulatory young adults age 20-25.  Facility also has a small geriatric dorm.



ORIENTATION:  Orientation Requirements: No call outs during orientation will be accepted. Restarting NEO or the ability to continue will be at the facility’s discretion. See documents attached on the Compliance Tab to share with candidates cleared for orientation.
 
PROFESSIONAL CONDUCT:  Facial piercings and cell phone usage are not permitted on any living area or during orientation. They will be dismissed.

LEAVE REQUESTS:  Once agency staff begin employment, no leave requests will be accepted during the first eight (8) weeks of employment.

DRIVING REQUIREMENT:
Contractors will be required to drive state vehicles transporting patients to outside facility doctor appointments and hospitals. Some may be required to be a "sitter" with the patient during an outside hospital stay.

MINIMUM QUALIFICATIONS:

1. Must be at least 18 years of age at time of hire.

2. Must hold unencumbered licensure as a Licensed Practical Nurse

3. One year of nursing experience, strongly preferred.

4. Must be CPR certified, or able to complete training.

5. Must be able to complete full orientation at time of hire.

6. Must be able to fulfill physical requirements of position including ability to lift and carry 20 lbs., remain alert while on duty, and work 16-hour shifts on occasion. 


CANDIDATE RESPONSIBILITIES:

1. Assess resident complaints and determine action, notifying supervisor when necessary.

2. Admit and transfer residents to and from other units or outside sources

3. Administer oral and/or injectable medication and controlled medications in accordance with facility policy, and within their scope of practice as defined by Florida law.

4. Observe for side effects or adverse effects to medication and report to provider for further assessment and treatment.

5. Prepare paperwork as required for consults within facility and outside appointments.

6. Coordinates residents’ appointments with unit staff to ensure medications are appropriately timed as need for resident movement to and from the unit.

7. Provide emergency medical/psychiatric treatment within scope of practice as defined by facility and Florida law and notifying supervisor for further assistance.

8. Complete incident reports factually and objectively, within the appropriate time frame as defined by the facility.
