We are seeking a compassionate and experienced RN Hospice Case Manager to provide exceptional end-of-life care to patients and families throughout Loudoun County, Virginia. The Hospice Case Manager is responsible for coordinating and managing patient care, conducting comprehensive assessments, developing individualized care plans, and ensuring patients receive comfort-focused care that supports dignity and quality of life.

Minimum Qualifications

• Active Virginia RN License (License must be in hand)
• Current BLS Certification
• Minimum 1 year of recent Hospice Nursing experience
• Valid Driver's License and reliable transportation
• Strong assessment, communication, and care coordination skills

Key Responsibilities

• Manage an assigned caseload of hospice patients.
• Conduct comprehensive patient assessments and develop individualized plans of care.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and other interdisciplinary team members.
• Provide symptom management and comfort-focused nursing interventions.
• Educate patients and caregivers regarding disease progression, medications, and end-of-life care.
• Monitor patient status and adjust care plans as needed.
• Maintain accurate, timely, and compliant clinical documentation.
• Communicate effectively with patients, families, physicians, and the interdisciplinary care team.
• Ensure continuity of care and advocate for patient needs throughout the hospice journey.
