Position Summary

We are seeking a dedicated Physical Therapist to provide patient-centered care in an outpatient setting, with occasional coverage in acute care. The Physical Therapist will evaluate patients, develop individualized treatment plans, implement therapeutic interventions, and collaborate with physicians and interdisciplinary team members to achieve optimal patient outcomes.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Provide therapeutic interventions to restore and improve functional mobility.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers regarding diagnoses, prognosis, and home exercise programs.
• Maintain accurate and timely documentation in compliance with regulatory standards.
• Collaborate with physicians and healthcare team members to coordinate patient care.
• Supervise Physical Therapist Assistants, aides, and students as required.
• Participate in interdisciplinary meetings and quality improvement initiatives.
• Promote patient safety and provide exceptional customer service.

Requirements

• Bachelor’s Degree in Physical Therapy required.
• Active Physical Therapist (PT) license required.
• BLS Certification preferred (within one month of hire).
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently and effectively within a multidisciplinary team.
• Experience in outpatient rehabilitation preferred.
