Position Overview:
Seeking a skilled and dependable Registered Nurse to provide high quality home health care and case management services. This role involves managing a caseload of patients while delivering a full range of home health visit types to support patient recovery, independence, and overall well being in the home setting.

Schedule & Work Details:

• Location: Lake, Ashtabula, and Geauga Counties
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hours: 40 hours, with a 36 hour guarantee
• On Call/Weekends: 1 week and 1 weekend per month
• Productivity Expectation: 30 units
• Pay Rate: $51 to $53 per hour
• Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities:

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
• Manage a patient caseload including coordination of care and ongoing case management responsibilities
• Perform all home health visit types including admissions, routine visits, recertifications, and discharges
• Conduct comprehensive assessments and monitor patient progress
• Educate patients and caregivers on disease management, medications, and safety practices
• Complete accurate and timely documentation of all visits and care provided
• Collaborate with interdisciplinary care teams to ensure continuity and quality of care
• Adjust care plans as needed to support optimal patient outcomes

Required Qualifications:

• Active Registered Nurse (RN) license in the state of Ohio
• Recent home health experience required
• Strong clinical and case management skills
• Ability to work independently and manage time effectively
• Valid driver’s license and reliable transportation

Preferred Qualifications:

• Experience with Homecare Homebase (HCHB)
• Strong communication and patient education skills
• Familiarity with home health best practices and regulatory standards
