Job Type: 13 Weeks Contract + possible extensions

Shift: Day 5x8-Hour (08:00 - 16:30)

Schedule: 5 days/week

Expected Hours: 40 hours per week

Job Overview

We are seeking a dedicated and compassionate Home Health Physical Therapist (PT) to provide high-quality patient care in a home setting. The ideal candidate will be responsible for evaluating patients, developing individualized treatment plans, and helping patients regain mobility and independence.

Key Responsibilities

• Perform patient assessments, evaluations, and reassessments in a home health setting
• Develop and implement individualized treatment plans based on patient needs
• Provide therapeutic interventions including mobility training, strengthening, and pain management
• Educate patients and families on home exercise programs, safety, and injury prevention
• Monitor patient progress and modify treatment plans accordingly
• Complete accurate and timely documentation including OASIS, care plans, and discharge summaries
• Collaborate with physicians and interdisciplinary teams to ensure coordinated care
• Utilize assistive devices and adaptive equipment as needed
• Maintain compliance with all regulatory and clinical standards

Required Qualifications

• Minimum 1 year of experience as a Physical Therapist
• BLS/CPR
• Physical Therapist OR License/Pending license accepted
• Oasis is required

  - Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)

  - Will this traveler need to float between like units?: No
  - Open to accommodating block schedule?: No
  - Locals Accepted: Yes; Considered "local" within 50-75 mile radius, will accept locals at the same rate
  - Pending License Accepted: Yes
  - RTO Restrictions: Confirm with the hiring manager
  - Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
