Home Health Registered Nurse (RN) – Multiple Locations Throughout New York State

Pay: $$58/hr
Schedule: Hybrid Flexibility (2 Days Remote, 3 Days Field Visits)
Benefits: Weekly Pay, Medical/Dental Insurance, $200 Travel Stipend, 3 Weeks PTO, 401(k) Match, Long-Term Contract with No End Date

We are actively hiring Home Health Registered Nurses (RNs) throughout New York State to conduct in-home assessments and support patients in accessing long-term care services.

Responsibilities:

• Perform in-home patient assessments.
• Review medical records and clinical documentation.
• Complete electronic UAS assessments.
• Document findings and maintain accurate case notes.
• Coordinate with healthcare teams and community resources.
• Educate patients on available care options and services.

Requirements:

• Active New York RN License.
• Minimum 2 years of Home Health, Home Care, or Assessment experience.
• Active NY Driver's License or State ID.
• Strong assessment, communication, and documentation skills.
