Travel RN Case Manager – Apple Valley, CA

🕒 Day Shift | 5x8s (8:00 AM – 4:30 PM)
We are seeking an experienced RN Case Manager for a travel assignment at a leading acute care hospital in California.
Requirements:
- Active California RN License
- Minimum 2 years of Acute Care Case Management and Discharge Planning experience
- Previous travel experience required (First-time travelers will not be considered)
- Epic experience required
- Recent hospital-based Case Management experience required
- Behavioral Health-only experience does not qualify
Key Responsibilities:
- Care Coordination and Discharge Planning
- Utilization Review
- Family Meetings and Treatment Planning
- DME, SNF, and Transportation Arrangements
- Prior Authorizations and Benefits Coordination
- Collaboration with interdisciplinary teams
Required Experience:
- Acute Care Hospital Setting
- Emergency Department, ICU, Med-Surg, or Surgical Services
- HMO/IPA Knowledge
- CMS and Regulatory Compliance
- Independent discharge planning and care coordination
