Description

Requirements:

Active New York RN License
Minimum 2 years of Home Health experience within the last 10 years
Experience with CHHA, LHCSA, MLTC, Hospice, Assisted Living, or Home & Community-Based Services
Recent experience conducting comprehensive patient assessments
BLS Certification
UAS-NY assessment experience strongly preferred
Experience with geriatric patients and chronic disease management preferred

Responsibilities:

Conduct in-home assessments and evaluations
Develop individualized long-term care service plans
Review medical records and referral documentation
Complete UAS assessments and electronic documentation
Coordinate care with interdisciplinary teams
Educate patients on available healthcare services and support options

Important Notes:

Candidates must be willing to perform both telehealth and field-based assessments
Cannot perform UAS assessment assignments with another employer simultaneously
Resume, Nursys Verification, NY License Number, and BLS certification are required for submission
