Description

Pride Health is seeking dedicated Hospice Registered Nurses to join our client's growing hospice team across multiple locations in Pennsylvania.

📍 Open Locations:
• Johnstown, PA 15904
• Plains, PA 18705
• Lewistown, PA 17044
• Berwick, PA 18603
• Danville, PA 17821

💰 Compensation & Benefits:
• Local Pay: $50–$55/hour (W-2)
• Travel Weekly Gross Pay: Up to $2,500/week
• Mileage Reimbursement: Yes

🕒 Schedule:
• Monday – Friday
• 8:00 AM – 4:30 PM
• 8-hour shifts
• Productivity: 5–6 patient visits per day
• On-Call Requirement: 2 nights per week (Monday–Thursday rotation)

🎯 Key Responsibilities:
Coordinate and deliver comprehensive hospice nursing care, including patient assessments, symptom and pain management, development and execution of individualized care plans, and ongoing monitoring of terminally ill patients in home or residential settings.
Serve as the primary liaison between patients, families, physicians, and the interdisciplinary hospice team by providing caregiver education, maintaining accurate EMR documentation, communicating changes in patient condition, and participating in care plan reviews and IDT meetings.

✅ Required Qualifications:
• Active Pennsylvania RN License
• Current CPR/BLS Certification
• Strong clinical assessment and patient care skills
